What Are Problems and Solutions?
A problem is a challenge, obstacle, or issue that hinders progress or creates difficulties in achieving a goal. Problems can be personal, professional, societal, or technical, and they often require critical thinking and decision-making to resolve.
A solution is an action, strategy, or method implemented to overcome a problem. Effective solutions address the root cause of the problem rather than just its symptoms and are designed to create long-term improvements.

The parable of the blind men and an elephant originated in the ancient Indian subcontinent, from where it has been widely diffused. It is a story of a group of blind men who have never come across an elephant before and who learn and conceptualize what the elephant is like by touching it. Each blind man feels a different part of the elephant's body, but only one part, such as the side or the tusk. They then describe the elephant based on their limited experience and their descriptions of the elephant are different from each other. In some versions, they come to suspect that the other person is dishonest and they come to blows. The moral of the parable is that humans have a tendency to claim absolute truth based on their limited, subjective experience as they ignore other people's limited, subjective experiences which may be equally true.[1][2]
The Buddhist text Udana 6.4,[3] contains one of the earliest versions of the story. It is dated to around c. 500 BCE, during the lifetime of the Buddha, although the parable is likely older than the Buddhist text.[4][better source needed]
An alternate version of the parable describes sighted men, experiencing a large statue on a dark night, or feeling a large object while being blindfolded. They then describe what it is they have experienced. In its various versions, it is a parable that has crossed between many religious traditions and is part of Jain, Hindu and Buddhist texts of 1st millennium CE or before.[5][4] The story also appears in 2nd millennium Sufi and Bahá’í lore. Organizations rely on people who can assess both kinds of situations and calmly identify solutions. Problem-solving skills are traits that enable you to do that. While problem-solving skills are valued by employers, they are also highly useful in other areas of life like relationship building and day-to-day decision making. There are five key steps of Problem-Solving which you will learn in this course and explained one by one, they are as follows
1. Define the Problem
The first step is to recognize and clearly define the problem. A well-defined problem sets the foundation for effective solutions.
Example: A sales team notices a sudden drop in revenue. Instead of assuming the cause, they analyze data and identify that customer churn has increased due to delayed deliveries.
2. Analyse Root Cause
Once the problem is identified, the next step is to investigate its root causes. This involves gathering data, identifying patterns, and understanding the factors influencing the issue.
Example: The sales team examines delivery processes and realizes that warehouse inefficiencies and supplier delays are leading to late shipments.
3. Develop Countermeasures
Brainstorming and evaluating multiple solutions help in selecting the most effective approach. These solutions should be practical and aligned with the organization's goals.
Example: The sales and logistics teams come up with potential solutions like automating inventory management, improving supplier coordination, and offering express delivery for high-priority customers.
4 . Implement Solutions
After selecting the best solution, implementation is key. A proper action plan with responsibilities and timelines ensures smooth implementation.
Example: The company upgrades its inventory management system, renegotiates supplier contracts for faster deliveries, and introduces a new express delivery option.
5 . Evaluate and Follow Up
The final step is to monitor the effectiveness of the solution by evaluation and follow ups and make necessary adjustments. Continuous assessment helps in refining strategies for better results.
Example: After a month, the company reviews delivery performance and customer feedback. If issues persist, further adjustments are made, such as adding more logistics partners or optimizing warehouse operations.

Reviewing and documenting how processes currently work (i.e., who does what, with what information, using what tools, communicating with what organizations and individuals, in what time frame, using what format).
Evaluating the possible impact of new tools and revised policies in the development of your "what should be" model.
A root cause is defined as a factor that caused a non-conformance and should be permanently eliminated through process improvement. The root cause is the core issue—the highest-level cause—that sets in motion the entire cause-and-effect reaction that ultimately leads to the problem(s).
Root cause analysis (RCA) is defined as a collective term that describes a wide range of approaches, tools, and techniques used to uncover causes of problems. Some RCA approaches are geared more toward identifying true root causes than others, some are more general problem-solving techniques, and others simply offer support for the core activity of root cause analysis.
HISTORY OF ROOT CAUSE ANALYSIS
Root cause analysis can be traced to the broader field of total quality management (TQM). TQM has developed in different directions, including a number of problem analysis, problem solving, and root cause analysis.
Root cause analysis is part of a more general problem-solving process and an integral part of continuous improvement. Because of this, root cause analysis is one of the core building blocks in an organization’s continuous improvement efforts. It's important to note that root cause analysis in itself will not produce any results; it must be made part of a larger problem-solving effort for quality improvement.
CONDUCTING ROOT CAUSE ANALYSIS
When carrying out root cause analysis methods and processes, it's important to note:
While many root cause analysis tools can be used by a single person, the outcome generally is better when a group of people work together to find the problem causes.
Those ultimately responsible for removing the identified root cause(s) should be prominent members of the analysis team that sets out to uncover them.
A typical design of a root cause analysis in an organization might follow these steps:
A decision is made to form a small team to conduct the root cause analysis.
Team members are selected from the business process/area of the organization that experiences the problem. The team might be supplemented by:
A line manager with decision authority to implement solutions
An internal customer from the process with problems
A quality improvement expert in the case where the other team members have little experience with this kind of work
The analysis lasts about two months. During the analysis, equal emphasis is placed on defining and understanding the problem, brainstorming its possible causes, analyzing causes and effects, and devising a solution to the problem.
During the analysis period, the team meets at least weekly, sometimes two or three times a week. The meetings are always kept short, at maximum two hours, and since they are meant to be creative in nature, the agenda is quite loose.
One person in the team is assigned the role of making sure the analysis progresses, or tasks are assigned to various members of the team.
Once the solution has been designed and the decision to implement has been taken, it can take anywhere from a day to several months before the change is complete, depending on what is involved in the implementation process.
The fishbone diagram or Ishikawa diagram is a cause-and-effect diagram that helps managers to track down the reasons for imperfections, variations, defects, or failures.
The diagram looks just like a fish’s skeleton with the problem at its head and the causes for the problem feeding into the spine. Once all the causes that underlie the problem have been identified, managers can start looking for solutions to ensure that the problem doesn’t become a recurring one.

Pareto Analysis is a statistical technique in decision-making used for the selection of a limited number of tasks that produce significant overall effect. It uses the Pareto Principle (also known as the 80/20 rule) the idea that by doing 20% of the work you can generate 80% of the benefit of doing the entire job. Take quality improvement, for example, a vast majority of problems (80%) are produced by a few key causes (20%). This technique is also called the vital few and the trivial many.
In the late 1940s Romanian-born American engineer and management consultant, Joseph M. Juran suggested the principle and named it after Italian economist Vilfredo Pareto, who observed that 80% of income in Italy went to 20% of the population. Pareto later carried out surveys in some other countries and found to his surprise that a similar distribution applied.
We can apply the 80/20 rule to almost anything:
80% of customer complaints arise from 20% of your products and services.
80% of delays in the schedule result from 20% of the possible causes of the delays.
20% of your products and services account for 80% of your profit.
20% of your sales force produces 80% of your company revenues.
20% of a systems defects cause 80% of its problems.
