
In this video, we look at the anatomy of the breast which includes the vascular supply and lymphatic drainage.
History taking involves looking at the history of the lump, lump changes, assessment of cancer risk, other risk factors for cancer and analysing the breast susceptibility gene (BRCA1 vs BRCA2 genes)
After positioning the patient, you will check the general appearance of the skin, look for any changes, Nipple discharge, palpate the breast, characterise the lump, palpate for lymph notes.
We look at the difference between painful and painless lumps and the the differential diagnoses for each category. We also look at the four most common types of breast lumps (age of onset, presence of pain, margins, consistency, mobility).
Discharge colours range from bloody to foul smelling pus. Causes differ per discharge and management of each category is essential. We also analyse pathological Nipple discharge, the history of it and the appropriate investigations which follow thereafter.
Within this topic, we will look at the histopathology (ductal vs lobular carcinoma in situ), clinical presentation, features suggestive of breast cancer, diagnosis by triple assessment, staging criteria, local vs haematogenous vs lymphatic spread and prognostic indices.
The 3 imaging modalities used to diagnose breast conditions are Ultrasoud, Mammogram and MRI. within each modality, we will look at the benign and malignant features. The MRI is the most sensitive imaging modality. Finally, we also look at the Bi-RADS classification, core needle biopsy and fine needle aspiration.
