History and Development of CST
Craniosacral Therapy was developed by Dr. John E. Upledger, an osteopathic physician, in the 1970s. While assisting in a spinal surgery, Dr. Upledger noticed rhythmic movements in the spinal cord, which he later identified as the craniosacral rhythm. This observation led him to further research and develop CST.
Dr. Upledger's work built on the earlier findings of Dr. William Sutherland, an osteopath who first recognized the movement of cranial bones in the early 20th century. Dr. Upledger expanded on these ideas, focusing on the role of the dura mater (the tough outer layer of the meninges) and its connections within the body.
What is Craniosacral Therapy (CST)?
Craniosacral Therapy (CST) is a gentle, hands-on technique that evaluates and enhances the functioning of the craniosacral system. This system includes the membranes and cerebrospinal fluid that surround and protect the brain and spinal cord. CST is based on the concept that the body's craniosacral rhythm, which is different from the cardiovascular and respiratory rhythms, can be felt and manipulated by a trained practitioner.
The therapy involves using light touch to assess the craniosacral system. The therapist then gently releases any restrictions they detect, which can improve the functioning of the central nervous system and promote overall health. CST is often used to address a variety of health issues, including chronic pain, headaches, stress, and more.

As we move into the palpation exercise, we'll focus on exploring the muscles, skin, and fascia. We will now begin the palpation exercise by gently placing our hands on the area to be explored. We will start with the skin, noticing its texture and temperature. As we gradually increase pressure, we'll feel the underlying fascia, observing any tension or smoothness. Finally, we'll palpate the muscles, sensing their tone and any areas of tightness. We'll take our time with each layer to deepen our connection with the tissues beneath our hands. This is really important to heighten our palpation skills!

Pay attention to the smoothness of motion and any restrictions or irregularities. Take note of the surrounding tissues and how they respond as you palpate.
We will now palpate the pulses. We will gently place our fingertips on the arteries, applying light pressure until we feel the rhythmic pulsation. We will focus on the strength, rhythm, and regularity of the pulse. We will take our time to tune into these subtle movements, noting any variations or changes in the pulse's quality.
We will now transition between palpating the pulses and breath. We will begin by feeling the pulse, noting its rhythm and strength. Then, we will shift our focus to the breath, placing our hands on the chest or abdomen to sense the rise and fall with each inhale and exhale. We will alternate between these two, observing how the pulse and breath interact, and how they influence each other in rhythm and flow.
We will now do a heartbeat awareness exercise feeling the sensation of the heartbeat through various areas of the body without using touch
This exercises was a favorite of mine as a massage therapist. You will feel the energy between the hands repelling each other like two magnets where a positive end meets a positive end of another magnet.
This is another one that I used constantly to center myself and also feel the energy flow throughout the front and back along my spine.
It's important in craniosacral therapy to have a solid understanding of anatomy. We will now focus on the cranium. We'll review the bones, sutures, and how they interrelate, as well as the cranial rhythms that play a key role in craniosacral work. Understanding these structures will enhance our ability to palpate and work effectively with the cranial system during therapy.
We will now look at the meninges. These protective membranes surround the brain and spinal cord, consisting of the dura mater, arachnoid mater, and pia mater. Understanding the structure and function of the meninges is crucial in craniosacral therapy, as they influence the flow of cerebrospinal fluid and the overall health of the central nervous system. We will explore their anatomy to better appreciate their role in craniosacral work.
We will now learn about the craniosacral rhythm. This subtle, rhythmic movement is generated by the fluctuations of cerebrospinal fluid within the craniosacral system. It's a key aspect of craniosacral therapy, guiding practitioners in assessing and addressing imbalances. We will explore how to palpate and tune into this rhythm, deepening our understanding of its significance in promoting health and well-being.
We will now learn to palpate the bones and sutures of the skull. We'll begin by gently placing our hands on the cranium, feeling for the different bones and their connections. We'll focus on identifying the sutures, the joints between the skull bones, noting any irregularities or tension. This practice will help us develop a more precise touch and a deeper understanding of cranial anatomy, essential skills in craniosacral therapy.
Entrainment is a phenomenon where the rhythms of two or more systems synchronize with each other. In the context of healing, particularly in practices like craniosacral therapy, entrainment occurs when the practitioner's calm, steady presence influences the client's bodily rhythms—such as their heartbeat, breathing, or craniosacral rhythm. This synchronization can create a state of deep relaxation and balance, allowing the body to access its natural healing processes more effectively. By tuning into and harmonizing with the client's rhythms, the practitioner supports the body's ability to restore health and well-being.
This study provides scientific evidence supporting the existence of the craniosacral rhythm, which has been a controversial topic in the osteopathic and broader medical communities.
The study titled "Direct Measurement of the Rhythmic Motions of the Human Head Identifies a Third Rhythm" by Thomas Rosenkilde Rasmussen and Karl Christian Meulengracht, presents evidence for the existence of a third cranial rhythm distinct from respiratory and arterial rhythms.
Key Findings:
-Existence of a Third Rhythm: The study found a distinct cranial rhythmic movement in all 50 participants, separate from the well-known respiratory and arterial rhythms. This third rhythm was consistently observed and had a mean frequency of 6.16 cycles per minute.
-Methodology: The researchers used a newly developed machine to measure the rhythmic oscillations of the head. The machine was able to detect movements with a sensitivity of 1 μM, ensuring accurate and objective measurement.
- Objective Evidence: Unlike previous studies that relied on palpation, this study used direct measurement techniques, providing objective evidence of the third rhythm's existence.
- Implications: This study supports the osteopathic concept of the primary respiratory mechanism (PRM) and the cranial rhythmic impulse (CRI), suggesting that this third rhythm could be an important physiological phenomenon.

This subtle movement is a key aspect of craniosacral therapy, reflecting the natural fluctuations of cerebrospinal fluid within the craniosacral system. To begin, we'll place our hands gently on the cranium tuning in with patience and sensitivity. As we focus, we'll start to perceive a gentle, rhythmic motion, distinct from the heartbeat and breath. This practice will help us connect with the body's natural rhythms, deepening our ability to support the healing process.

Research into cranial suture motion has evolved significantly as cranial osteopathy and craniosacral therapy have gained popularity and broader acceptance. Early in medical history, the prevailing view was that cranial sutures fuse and become immobile as we reach adulthood. This belief shaped much of conventional medicine's approach to cranial anatomy. However, as these therapies developed, practitioners began to suggest that cranial bones could exhibit subtle movement well beyond childhood.
Thanks to modern imaging techniques and more in-depth studies, we now have growing evidence to support the idea that cranial sutures do, in fact, maintain subtle motion throughout life. This motion, though small, is vital for overall craniosacral health and cerebrospinal fluid circulation.
What’s in the cranial sutures?
Cranial sutures are complex structures composed of fibrous tissue, blood vessels, nerve endings, osteogenic cells, and attachments to the dura mater. These components work together to maintain the flexibility, stability, and health of the skull, allowing for slight cranial motion while protecting the brain. This subtle mobility in the sutures plays a crucial role in craniosacral therapy, where detecting and releasing tension in these areas can promote balance and healing throughout the body.
The reciprocal tension membranes are essential to craniosacral therapy, as they regulate and maintain the balance between the cranial bones. By treating restrictions and tension in these membranes, therapists can restore the natural movement of the cranial bones, relieve symptoms of cranial dysfunction, and promote overall balance within the craniosacral system. Understanding the intricate relationship between the RTMs and the cranial bones is key to effective treatment.
The falx cerebri and tentorium cerebelli exert significant influence on the internal cranial structures due to their deep attachments. Their connections to the cranial bones mean that any tension, restriction, or imbalance in these membranes can create pulling forces within the skull, affecting both the cranial bones and the soft tissues they surround. As you can see in this lecture, the pull creates lines of stress on the bones reshaping them..
This shows you the real amount of stress on the bony structures over the years.
When there are restrictions in the cranial bones, like the occiput, sphenoid, ethmoid, or temporal bones, it can lead to tension in the reciprocal tension membranes—the falx and tentorium—since they attach directly to these bones. This, in turn, impacts the structures they surround, including the brain and spinal cord. Similarly, restrictions within these membranes can affect the bones they attach to, potentially leading to symptoms such as headaches, visual disturbances, balance issues, and neck tension.
In the early stages of palpating cranial structures, motion, or cranial rhythms, it is important to adjust your expectations to the subtleties of the system. Rather than anticipating gross, obvious movement, focus on detecting a more delicate and nuanced sensation.
Recognizing the Cranial Rhythm:
As you settle into the palpation, this sensation of fullness may intensify slightly, then after a few seconds, you may notice a receding of the surge, akin to the ‘tide going out’. This ebb and flow is a delicate, yet unmistakable aspect of craniosacral rhythm. Once you’ve tuned into it, it becomes easier to detect and follow the rhythm in various parts of the body. The key is to be patient and sensitive to these subtle motions, resisting the temptation to expect more pronounced movements typical in other types of manual therapy.
Palpating for Motion at Cranial Sutures:
When evaluating the motion of individual bones or sutures, it is critical to maintain the same soft awareness. The normal motion you feel under your fingers will be a sense of pliability—the bone or suture will have a degree of yielding, almost as though it is accommodating the pressure of your touch. The tissue should feel responsive, and you may notice a slight ‘give’ as the bones subtly adjust within their natural range of motion.
Recognizing Restriction:
In contrast, when a cranial bone or suture is restricted, the sensation will be quite different. Instead of the pliable, responsive feeling, you will notice a hard, wooden, or unyielding sensation under your fingers. This restricted area may feel as though it is resisting movement, lacking the natural flexibility and motion that indicate healthy function. A loss of motion potential at a suture, for example, feels blocked—as though the bone is unable to engage in the rhythmic flow of the craniosacral system. In such cases, the tissue may feel tense or stiff, signaling an area of dysfunction that requires gentle release or decompression.
Now it's time to dive in and start feeling the sutures! This is where your hands-on skills really come into play. As we move through the next phase, you'll learn to palpate the cranial sutures, becoming attuned to any restrictions or areas of tension.
Whether or not you can feel the craniosacral rhythm, you’ll still be able to locate joint restrictions by tuning into the subtle differences in the sutures' mobility and texture. Take your time, let your hands explore, and begin to notice where movement feels restricted or stuck. This is where the release techniques come into action, helping to restore balance and freedom to the cranial system.

Now, we’re going to begin with the Occipitomastoid Suture Spring Test.
This is a palpation exercise designed to help you get a feel for the motion at the junction between the occipital and temporal bones. By gently testing the mobility of the occipitomastoid suture, you'll not only identify any restrictions, but the act of palpating can also help free up those restrictions. These exercises are both diagnostic and therapeutic, allowing you to develop your touch while promoting balance within the craniosacral system
The Occipitomastoid Suture Spring Test is primarily used to practice palpation skills in this lecture, but it can also provide therapeutic benefits by assessing and releasing restrictions between the occiput and the mastoid portion of the temporal bone. Releasing restrictions in this area can help with a variety of conditions, particularly those related to the cranial nerves and the balance of the head and neck.
Conditions Benefited by the Occipitomastoid Suture Spring Test:
1. Headaches and Migraines: Tension or restriction at the occipitomastoid suture can cause or exacerbate headaches, especially those originating at the base of the skull. Releasing this tension can alleviate headache symptoms.
2. Neck Pain and Upper Cervical Tension: The occipitomastoid suture is located at the junction of the skull and upper cervical spine. Releasing restrictions here can help relieve neck pain and tension in the upper cervical region.
3. Balance and Dizziness (Vertigo): The mastoid process houses part of the vestibular system, which is responsible for balance. Dysfunction at the occipitomastoid suture can affect balance, and releasing it may help alleviate dizziness or vertigo.
4. Tinnitus and Ear Issues: The temporal bone is directly involved in hearing and balance. Restrictions at the occipitomastoid suture can contribute to tinnitus, ear pain, or pressure, and releasing this tension may help alleviate these symptoms.
5. Cranial Nerve Dysfunction: Several cranial nerves pass through or near the occipitomastoid region, including the accessory nerve (CN XI) and the vagus nerve (CN X). Releasing restrictions in this area can help relieve cranial nerve compression, which can manifest as neck pain, swallowing difficulties, or even voice changes.
6. TMJ Dysfunction: The occipitomastoid suture impacts the temporal bone, which is part of the temporomandibular joint (TMJ). Releasing tension here can indirectly help with TMJ dysfunction, jaw pain, and clicking.
7. Upper Body Tension and Postural Imbalances: Dysfunction at the occipitomastoid suture can affect the alignment of the entire spine and contribute to upper body tension. Releasing this area can improve overall postural alignment and relieve tension in the shoulders and upper back.
The Occipitomastoid Suture Spring Test not only enhances palpation skills but also serves as a valuable tool for releasing cranial tension and restoring mobility, especially in the cranial base. By improving function in this area, it can alleviate a variety of conditions related to the cranial nerves, neck tension, balance, and TMJ dysfunction.
Next, we’ll move on to the Parietotemporal Suture Spring Test.
This palpation exercise focuses on the junction between the parietal and temporal bones, an area that is crucial for overall cranial mobility. As you gently test the motion at the parietotemporal suture, you’ll begin to feel for any restrictions or areas that may lack normal movement. Just like with the previous test, the simple act of palpating can help release restrictions while giving you a better sense of the cranial dynamics. These exercises serve a dual purpose—both helping you learn to feel subtle movements and freeing up any stuck areas in the sutures.
Now, we’re going to explore the Sphenotemporal Suture Spring Test.
This palpation exercise focuses on the junction between the sphenoid and temporal bones, which plays a critical role in cranial function and balance. As you apply gentle pressure to test the motion at the sphenotemporal suture, you’ll be assessing for restrictions or immobility. At the same time, this exercise can also help free up any tension or restrictions, making it both a diagnostic and therapeutic tool. Use this test to refine your palpation skills and to support the natural movement of the cranial sutures.
Conditions Benefited by the Sphenotemporal Suture Spring Test:
1. Headaches and Migraines: Tension or restriction at the sphenotemporal suture can contribute to headaches, particularly those related to pressure at the cranial base. Releasing this tension can alleviate headache symptoms.
2. TMJ Dysfunction (Jaw Pain and Clicking): Restrictions in the sphenotemporal suture can cause or exacerbate TMJ dysfunction, leading to jaw pain, clicking, or difficulty chewing. Releasing this suture can help alleviate TMJ symptoms.
3. Sinus Congestion and Facial Pain: The sphenoid bone is central to the cranial structure and affects sinus drainage and can contribute to sinus issues, and releasing this area may help improve sinus drainage and relieve facial pain or pressure.
4. Ear Issues (Tinnitus, Hearing Problems): The temporal bone houses the inner ear structures, and tension at the sphenotemporal suture can impact hearing and contribute to tinnitus or ear pressure. Releasing restrictions in this area can improve ear function and relieve symptoms.
5. Eye Strain and Visual Disturbances: The sphenoid bone plays a crucial role in supporting the eye sockets. Restrictions in its movement can contribute to eye strain, blurry vision, or visual disturbances. Freeing up the sphenotemporal suture can relieve tension in the sphenoid and improve eye function.
6. Dizziness and Balance Issues (Vertigo): The temporal bone is part of the vestibular system, which controls balance. Dysfunction at the sphenotemporal suture can affect balance and cause dizziness or vertigo. Releasing this area can help alleviate balance-related problems.
7. Cranial Nerve Dysfunction: The sphenoid and temporal bones are key areas for several cranial nerves, including those related to eye movement, swallowing, and facial sensation.
8. Neck and Shoulder Tension: Restrictions in the sphenotemporal suture can contribute to tension throughout the neck and shoulders. Releasing this area can improve postural alignment and reduce muscle tension in the upper body.
In cranial manipulative therapy, recognizing when a release has occurred is an essential skill. A release refers to the moment when restricted tissues—such as cranial sutures, fascia, or membranes—let go of tension, allowing for improved mobility, fluid flow, and overall function.
In cranial manipulative therapy, signs of a release include:
- Change in tissue texture or tone (softening, more pliability)
- Subtle movement or shift in cranial bones or tissues
- Balanced or stronger craniosacral rhythm

If you’re able to feel the craniosacral rhythm, it can serve as a helpful tool for identifying imbalances and guiding your treatment. By first palpating the rhythm, you can sense areas of restriction or dysfunction. After applying the techniques throughout this course to release cranial suture restrictions, you can then re-evaluate the rhythm to see if it has become more balanced, providing a clear indication of progress.
However, it's important to note that if you cannot feel the craniosacral rhythm, that's perfectly fine—it's not necessary to achieve effective results. You can still apply the techniques by palpating for joint restrictions and working to release them, using your hands to sense the areas of tension or immobility. Whether or not you feel the rhythm, these techniques will still help restore motion and balance in the craniosacral system.

Both the compression and decompression of the fronto-occipital structures work to adjust the relationship between these key cranial bones. They balance cranial tissues and provide lengthening and relaxation, particularly affecting the reciprocal tension membranes such as the falx cerebri. These techniques can be beneficial in treating symptoms such as headaches, neck pain, and cranial tension
Conditions Benefited by Fronto-Occipital Decompression:
1. Headaches and Migraines: It helps relieve tension and pressure in the head, reducing the frequency and intensity of headaches, including migraines.

Fronto-Occipital Decompression is a versatile technique that supports the overall balance of the craniosacral system, making it useful for a wide range of conditions related to tension, circulation, and cranial nerve function.
The occipitosphenoidal translation technique (also referred to as a "shunt") is used to assess the movement and balance between the occipital bone and the sphenoid bone. This subtle cranial manipulation evaluates the ability of the sphenoid to shift laterally, helping identify restrictions or imbalances that may affect the craniosacral system.
Conditions Benefited by Occipitosphenoidal Translation (Shunt):
1. Headaches and Migraines: This technique helps reduce tension and restrictions at the cranial base, relieving pressure that can contribute to chronic headaches and migraines.
2. TMJ Dysfunction: Since the sphenoid bone is involved in the alignment of the jaw, releasing restrictions at the occipitosphenoidal junction can improve TMJ function and alleviate associated jaw pain and clicking.
3. Sinus Issues and Congestion: The sphenoid is central to the cranial structure, including sinus drainage. Mobilizing the occipitosphenoidal junction can promote better sinus drainage and relieve congestion.
4. Eye Strain and Visual Disturbances: The sphenoid bone is directly related to the orbits of the eyes. Mobilizing the sphenoid relative to the occiput can alleviate tension contributing to eye strain, blurry vision, or visual disturbances.
5. Neck Pain and Tension: Releasing restrictions in the occipitosphenoidal junction can help resolve upper cervical tension and neck pain by restoring proper alignment at the cranial base.
6. Cognitive Issues (Brain Fog, Difficulty Concentrating): The occiput and sphenoid bones are involved in the flow of cerebrospinal fluid (CSF). Improving their alignment helps promote better CSF circulation, which can alleviate cognitive issues such as brain fog or difficulty concentrating.
7. Balance and Dizziness (Vertigo): Since the cranial base houses important structures related to balance, such as the vestibular system, this technique can help alleviate vertigo or balance-related problems.
8. Cranial Nerve Dysfunction: The occiput and sphenoid are integral to cranial nerve function, including nerves related to vision, swallowing, and hearing. Mobilizing this area can help reduce impingement on these nerves, improving symptoms like facial nerve pain, swallowing issues, or tinnitus.
The V-spread technique is a gentle craniosacral therapy method used to release restrictions in cranial sutures. By applying light pressure across a restricted suture, this technique helps to create a release in the underlying tissues, encouraging better movement and function within the craniosacral system. It is thought to work by relaxing the fascial reciprocal tension membranes connected to the sutures.
When approaching cranial conditions in craniosacral therapy, it's important to remember the phrase "hooves coming down the road—think horse, not zebra." This means that while cranial issues can arise from complex or subtle imbalances, they are often caused by the most obvious restrictions.
