In support of improving patient care, this activity has been planned and implemented by Amedco LLC and SEB Performance Improvement Consulting. Amedco LLC is jointly accredited by the Accreditation Council for Continuing Medical Education (ACCME), the Accreditation Council for Pharmacy Education (ACPE), and the American Nurses Credentialing Center (ANCC), to Provide continuing education for the healthcare team.

a Jointly Accredited Organization, Amedco is approved to offer social work continuing education by the Association of Social Work Boards (ASWB) Approved Continuing Education (ACE) program. Organizations, not individual courses, are approved under this program. State and provincial regulatory boards have the final authority to determine whether an individual course may be accepted for continuing education credit. Amedco maintains responsibility for this course. Social workers completing this course receive 1.75 GENERAL continuing education credits.

Instructor
Sharon Bratcher, M.Ed., BSN, RN is a life-long resident of Baltimore with significant connections to community healthcare and FQHCs. She has participated in several HRSA Operational Site Visits, TJC and NCQA accreditations. During her 29 years of experience as a nurse she has held positions in the arenas of Care Coordination, Care Management, Quality and Performance Improvement. She has a steadfast commitment to working collaboratively with organizations to promote improvement in health outcomes, safety, and process improvement.
Holding a Masters in Adult Education has enhanced her ability to use adult learning principles when designing and delivering a wide variety of trainings. This skill enables the transfer of knowledge to persons of varied backgrounds. She holds the Certified Professional in Healthcare Quality and Certified Diabetes Educator credential.
Ms. Bratcher understands how to foster the process of patient activation. She specializes in the use of motivational interviewing techniques when assisting patients with chronic illnesses to develop self- management skills. Ms. Bratcher is passionate about delivering healthcare with a patient-focused approach which produces sustained improvement in healthcare outcomes. She has been an effective role model and champion for patient empowerment education models which focus on collaborative solutions leading to enhanced self-care, a tenant of chronic disease management.
A few highlights of her career include holding positions as a Performance Improvement Coordinator at UMMC-Midtown Campus and the Department of Defense, Senior Clinical Quality Nurse Consultant at United Healthcare, Manager, Care Coordination for the West Baltimore Health Enterprise Zone Collaborative, Cardiovascular Nurse Educator and Interim Care Management Program Manager at Baltimore Medical Systems, Manager of Clinical Development and Training at Med Assurant, Inc., and Diabetes Nurse Educator/Research Study Coordinator at the Joslin Diabetes Center.
