Background
The Basel Statements on the Future of Hospital Pharmacy provide an international vision for the advancement of hospital pharmacy practice. Developed through the International Pharmaceutical Federation (FIP), they describe important principles for strengthening hospital pharmacy services, improving medicines-use systems, supporting patient safety, and advancing the professional contribution of hospital pharmacists within healthcare systems.
Since their original development in Basel in 2008, the Statements have continued to evolve alongside changes in hospital pharmacy and healthcare. They were revised in Bangkok in 2016 and updated again in Brisbane in 2024, reflecting emerging priorities, expanding professional responsibilities, technological developments, workforce needs, medicines governance, and contemporary approaches to safe and effective medicines use.
The evolution from Basel (2008) to Bangkok (2016) and Brisbane (2024) demonstrates that the Basel Statements are not a static set of recommendations. Rather, they represent an evolving international framework intended to remain relevant to the changing needs of hospital pharmacy practice.
The 2024 updated Basel Statements address seven major domains:
Overarching and Governance
Procurement
Influences on Prescribing
Preparation and Delivery of Medicines
Administration
Monitoring of Medicines Use
Human Resources, Training, and Development
These domains span the medicines-use process and highlight the contribution of hospital pharmacists to areas such as medication safety, medicines governance, clinical decision-making, quality improvement, professional competency, interprofessional collaboration, and patient-centred care.
The Basel Statements have also been used internationally to support the assessment, benchmarking, and advancement of hospital pharmacy practice across different healthcare systems. Their broad scope makes them particularly valuable not only for understanding what high-quality hospital pharmacy should aim to achieve, but also for considering how these principles can be applied when healthcare professionals encounter complex practice situations.
This is where case-based learning becomes particularly valuable.
The present advanced practice-test course uses complex hospital pharmacy cases to challenge learners to move beyond recognizing Basel Statement principles toward applying them in realistic situations. Participants are required to interpret case information, identify key problems, evaluate alternative actions, consider patient and system-level implications, and select the most appropriate response.
Course Objectives
The present practice-test course aims to strengthen learners' ability to apply the 2024 updated Basel Statements through advanced case-based assessment.
By working through complex hospital pharmacy cases, participants will:
Strengthen their understanding of the 2024 updated Basel Statements and their practical implications for hospital pharmacy.
Apply Basel Statement principles to complex and realistic hospital pharmacy situations.
Develop critical thinking, analytical reasoning, decision-making, and problem-solving skills.
Evaluate challenges involving governance, procurement, prescribing, preparation and delivery, administration, monitoring of medicines use, and workforce development.
Analyze situations involving medication safety, quality improvement, professional responsibilities, and medicines-use systems.
Consider patient-specific, clinical, organizational, and professional factors when evaluating possible courses of action.
Reinforce the importance of evidence-based, patient-centred, collaborative, and responsible medicines use.
Use case-based assessment and feedback to identify strengths, knowledge gaps, and opportunities for further professional development.
Course Outline
The course consists of six advanced case-based practice tests covering the seven major domains of the 2024 updated Basel Statements.
Module 1: Overarching and Governance Statements
This module presents complex cases related to the broader responsibilities and systems that support hospital pharmacy practice. Learners evaluate situations involving governance, professional accountability, medicines-use systems, patient safety, quality improvement, organizational responsibilities, and the role of hospital pharmacists in supporting safe and effective care.
Module 2: Procurement
This module challenges learners with cases involving the selection, procurement, quality, availability, and responsible acquisition of medicines and medicine-related technologies. Participants consider issues such as procurement governance, quality assurance, supply-chain challenges, formulary processes, resource allocation, sustainability, and responsible decision-making.
Module 3: Influences on Prescribing
Cases in this module examine the different factors that influence prescribing and therapeutic decision-making. Learners apply principles related to evidence-based medicines use, patient-specific considerations, pharmacist involvement in prescribing decisions, formulary governance, documentation, medicines management, and collaborative practice.
Module 4: Preparation and Delivery
This module focuses on complex situations involving the preparation, compounding, handling, storage, labelling, distribution, and delivery of medicines. Learners assess potential risks and consider appropriate approaches to quality assurance, medication-error prevention, and safe medicine preparation and distribution.
Module 5: Administration
Cases address challenges surrounding the safe administration of medicines, including medication safety, interdisciplinary responsibilities, communication, documentation, patient information, and error prevention. Learners evaluate situations and determine approaches that support safer medicines administration.
Module 6: Monitoring of Medicines Use & Human Resources, Training and Development
The final module covers two major Basel Statement domains.
Cases related to Monitoring of Medicines Use address medication outcomes, medication safety, evaluation of medicines use, and opportunities for improving therapy and medicines-use systems.
Cases related to Human Resources, Training, and Development address workforce competency, education, training, professional development, staffing considerations, and the preparation of hospital pharmacy personnel to meet evolving professional responsibilities.
Together, the six practice tests provide opportunities to apply Basel Statement principles across the major components of contemporary hospital pharmacy practice.
Advanced Case-Based Assessment
The questions in this course are designed to require more than straightforward recall. Learners may need to interpret several pieces of information, recognize the most important problem, identify potential risks, compare alternative interventions, and determine the most appropriate response.
Through this approach, participants can practice moving from:
Knowledge → Interpretation → Analysis → Decision-Making → Application
Immediate feedback supports reflection on each response and allows learners to identify areas requiring additional study. The practice tests can therefore be used both to assess existing understanding and reinforce the application of Basel Statement principles.
Conclusion
The Basel Statements provide an international framework for envisioning and advancing high-quality hospital pharmacy practice. Their development from Basel (2008), through Bangkok (2016), to Brisbane (2024) reflects the continuing evolution of the profession and the increasing complexity of medicines use within modern healthcare systems.
Understanding these principles is important, but professional practice also requires the ability to apply them when decisions are complex and multiple priorities must be considered.
Applying the Basel Statements: Advanced Case-Based Learning provides an opportunity to develop this higher level of application through six advanced practice tests. By analyzing challenging hospital pharmacy cases, learners can strengthen their ability to connect the 2024 updated Basel Statements with practical decision-making, medication safety, quality improvement, and responsible medicines use.
